Job Title: Wound Care/MDS Nurse (RN or LVN) (Primary Wound Care Focus)

Reports to: Director of Nursing Services

Department: Nursing

Classification: Salaried (Exempt)

POSITION SUMMARY:

This dual-role position focuses primarily on wound care while also supporting MDS completion as needed. The Wound Care & MDS Nurse is responsible for delivering skilled wound treatment and skin care management to residents and assisting in completing the Resident Assessment Instrument (RAI), Minimum Data Set (MDS), and related documentation in compliance with State and Federal guidelines.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Primary - Wound Care Nurse Responsibilities:

Assess, plan, implement, and evaluate wound care treatment for residents.

Identify, manage, and treat skin disorders including ulcers, infections, lesions, calluses, and other dermatologic conditions.

Monitor and document resident skin conditions and response to treatments.

Communicate with physicians, residents, families, and interdisciplinary team members regarding skin and wound care.

Participate in resident care planning and revise treatments as needed based on clinical evaluations.

Maintain accurate and timely documentation in compliance with facility protocols and regulations.

Ensure appropriate supplies and equipment are stocked and available.

Secondary - MDS Nurse Responsibilities:

Assist with MDS assessments, including ARD scheduling, data collection, and documentation.

Complete assigned sections of MDS, CAAs, and care plans.

Support care plan meetings and ensure interdisciplinary coordination.

Monitor medical records for documentation accuracy related to MDS coding.

Participate in survey readiness and QAPI activities.

Provide staff education on documentation and ADL scoring, in collaboration with the DON and SDC.

QUALIFICATIONS:

Education and Experience:

Graduate of an accredited LVN/LPN or RN program.

Minimum 1 year of experience in long-term care or a related healthcare setting.

Prior wound care and MDS experience preferred.

Licensure and Certification:

Must hold a current, valid LVN/LPN or RN license in the State of Texas.

Wound care certification (WCC, WOCN, etc.) preferred but not required.

MDS certification a plus.

Additional Requirements:

Must be TB-free upon hire and comply with facility health policies.

Strong knowledge of long-term care regulations and nursing best practices.

Ability to make independent decisions and work collaboratively within an interdisciplinary team.

Must have a positive, compassionate attitude with strong communication and organizational skills.

Available to be on call as needed

WORK ENVIRONMENT & PHYSICAL REQUIREMENTS:

Frequent movement throughout the facility, including resident rooms.

Exposure to infectious diseases, odors, and hazardous chemicals.

Must be able to lift up to 25 lbs regularly and occasionally up to 50 lbs.

Required to respond during emergencies and work flexible hours, including weekends or holidays as needed.
