Wellness and Family Support Case Manager (Personal Services)

Scope and responsibilities:
The Wellness & Family Support Case Manager is responsible for managing the delivery of behavioral health services, providing guidance to the students and families we serve. This role will ensure consistency of standards within the Agencys Service Model so that services and programs express the policies and procedures of the Little Bit Foundation.
Program Implementation:
Ensure the provision of relevant, competent service to all children, families, and communities who either request or are determined to require assistance with their daily living from the initial point of contact to the conclusion. Ensure the assignment and consistent, active involvement of all assigned staff to build and maintain relationships with School District partners, including supporting existing partners, and navigating future growth opportunities.Completes and records initial and ongoing assessments and service plans for all assigned cases. Represent the Little Bit Foundation at community meetings, School District collaboration meetings, and care team meetings in partner schools.Oversee and provide school and community-based case management, skills training/development, and other services to children and their families, and ensure quality measures are met.Support and consult with school-based case managers as needed to assist with time management, documentation, and adherence to agency standards.Carry a limited caseload, as needed for any gaps in services, and acquire additional resources to support service delivery as needed.Observation and review of direct services performed by the SAFE team, to ensure staff is demonstrating the necessary skills required to correctly provide service and support to our schools and partners. At a minimum, complete a six (6) month and annual review for services and submit any recommendations for changes to the VP of Compliance and Programming.Other duties as assigned.
Education:
MSW or Master of Health Administration required. LCSW or MSW preferred.Sound knowledge of human behavior theories, child development, family dynamics, and the ability to apply knowledge in assessment and planning.
