CARDIO PULMONARY REHABILITATION NURSE
Job Summary:
The Cardio Pulmonary Rehabilitation Nurse is responsible for delivering specialized nursing care to patients with cardiac and pulmonary conditions who are enrolled in a rehabilitation program. This role involves developing and implementing individualized care plans, monitoring patient progress, educating patients and families, and promoting lifestyle modifications to enhance cardiovascular and respiratory health.
Supervisory Responsibilities:
Duties/Responsibilities:
Assess and evaluate patients referred to the cardiopulmonary rehab program.
Develop individualized exercise and education plans tailored to patient needs and clinical goals.
Monitor patients during exercise sessions, observing for signs of distress and responding appropriately to emergencies.
Perform routine vital sign checks (e.g., heart rate, blood pressure, oxygen saturation) and interpret telemetry/ECG data during rehab sessions.
Provide patient and family education on heart and lung disease management, medication adherence, nutrition, smoking cessation, and stress reduction.
Collaborate with a multidisciplinary team, including physicians, respiratory therapists, dietitians, and physical therapists.
Maintain accurate and timely documentation in patient records and prepare progress reports for referring providers.
Participate in quality improvement initiatives and ensure compliance with safety standards and rehabilitation protocols.
Support program development and outreach activities as needed.
Adheres to the facilitys Standard of Excellence.
