Hanford Post Acute is seeking a Part-Time Physical Therapist to join our dedicated rehabilitation team. This role is ideal for a clinically skilled PT who is passionate about delivering high-quality therapy services in a post-acute care setting.

Responsibilities:

  • Evaluate patients within 24 hours or 1 business day of physician referral; document cause for any delay
  • Develop individualized treatment plans and obtain physician approval
  • Deliver care as outlined in the treatment plan
  • Supervise Physical Therapy Assistants in patient care and related activities in compliance with the state practice act
  • Communicate with interdisciplinary team regarding patient progress and care planning
  • Participate in patient care conferences, utilization reviews, and in-service training
  • Maintain accurate and timely documentation of evaluations, daily notes, progress updates, and recertifications in accordance with PT Board and payer requirements
  • Instruct caregivers and nursing staff on maintenance programs and post-discharge care
  • Recommend and assist in ordering durable medical equipment to enhance patient mobility and independence
  • Report and monitor maintenance needs of therapy equipment
  • Support nursing with Restorative Aide training

Supervisory Responsibilities:
  • Supervise PT assistants, aides, and students as applicable

Qualifications:
  • Bachelor's degree in Physical Therapy required; Master's or Doctorate preferred
  • Current California PT license in good standing
  • CPR certification and ongoing CEUs as required by the state
  • Strong documentation skills, preferably with experience in Casamba and PointClickCare
  • Experience in SNF or post-acute care preferred
  • Ability to work collaboratively and maintain positive relationships with residents, families, and staff

Physical Demands:
  • Frequent standing, walking, reaching, and lifting (up to 100 lbs occasionally)
  • Occasional stooping, kneeling, or crawling
  • Vision requirements include close, color, peripheral, and depth perception

Work Environment:
  • Low to moderate noise level typical of a clinical setting
  • Reasonable accommodations available as needed
